Note:  We are currently at full capacity, and not taking new runners.  Please send us an email if you want to be put on a waiting list, and only fill out this questionnaire if you have been asked to do so by one of our coaches.

This questionnaire is completely no-obligation.  We use your responses to help determine if there is a fit between you as a runner and us as coaches.  We will respond via email after evaluating your responses, and let you know what we propose.  If you agree to proceed, then we will forward payment details at that time.

You may leave any of this information blank at this time (phone number and address for example) if you don't feel comfortable answering at this point.  Prior to beginning official coaching, however, the more information you can give us, the better we can tailor the training to your needs.
